| How the BCMS Welfare Database Was Researched and Some Qualifications That Apply: |

This database is designed to show market share by benefits type, brokers and carriers.
It further identifies the plan sponsors insured by and brokered by a particular
carrier and broker. It is updated semi-annually (Fall and Spring) and includes the
following data elements:

The data is taken from Schedule A of the Form 5500 (latest year available).
Many Schedule A forms report multiple policies,
and each policy is treated separately in the BCMS Welfare database.
Exclusions: Most welfare benefit plans with less than 100 participants are excluded,
as they do not have to file, however some still file due to various provisions in ERISA law.
Not every client (plan sponsor) of every carrier and broker is represented here,
only those made available by the Department of Labor in its latest public release.
Premiums & Participants: We have occasionally found premiums
reported to be higher when compared to the number of participants covered than they should be
(for example 90 people covered for Health with a premium of $44 million) and vice versa.
This is usually due to errors in the Department of Labor electronic scanning system.
Broker names and names of carriers: The DOL scans in the data and often it is
unintelligible or difficult to discern. We assembled a team of researchers to clean
up this data, and eliminated many errors and elements that were illegible. Please note:
Only the names and addresses of companies have been updated, the names and addresses of individuals are unchanged.
A special word about broker names: The Schedule A of the form 5500 allows only
one line and 35 characters for the broker name. There is no room to put an individual's
name as well as a company name, so the brokers tend to fall into two categories:
those with individual names such as Robert J. Brown and those with company names
such as Torrey & Clark Insurance Agency. All names were sent through the National
Change of Address System to ensure accuracy and standardization of the addresses.
Phone numbers were added where available. Since the broker names are from
2003 and 2004 filings, some brokers may now have merged or changed their name.
We have not been able to update addresses of brokers whose entries in the database are by individual name.
Those with company names have been scrubbed to the best of our ability through matching with a large
commercial database as well as through personal phone calls. We continue
to research and correct these names. Because the information is in an online database,
we are able to provide updates and corrections on a continuing basis.

Although most broker names and addresses have been cleaned up to the best of our ability,
we have provided brokers with a screen to update their address/phone information.
